- Aadhaar copy
- Cow’s ear-tag / microchip identification certificate and number
- Details of the cow’s active insurance
- Bank passbook copy or cancelled cheque
- Full photograph of the keeper with the cow
- Notarised affidavit that the cow has never received this award and is in its 1st, 2nd or 3rd calving
How to apply
- Apply within 45 days of calving in the prescribed form (Annexure-2) on the Nand Baba Milk Mission portal, or offline at the Chief Development Officer / Chief Veterinary Officer’s office (paras 6(iv), 7(i)).
- A local committee measures the cow’s milk over 4 milkings on 2 days (para 8(ii)); the award follows the Annexure-1 benchmarks.
