Papers you need
- Claim application on the prescribed form, verified by the Panchayat Pradhan or Secretary (cl. 7(a)(i)–(ii))
- Death: legal heir certificate, post-mortem report and death certificate (cl. 7(a)(iii)–(iv))
- Disability: medical certificate from a registered qualified doctor on the prescribed form, and a photograph of the remaining limb (Citizen’s Charter 2024-25, item 7)
How to apply
- Fill the claim form (Annexure A) — the legal heir signs it in case of death, the injured person in case of disability (cl. 7).
- Get it verified by the Panchayat Pradhan or Secretary (in a town, by the municipal officer) (cl. 7(a)(ii)).
- Submit it with the papers to the Subject Matter Specialist (Agriculture) of your block within 2 months; he sends it to the Deputy Director of Agriculture (cl. 7(a)(v)).
- The Deputy Director pays within 15 days of receiving a complete claim (cl. 7(a)(vi)).
Applying is free. Never pay anyone to apply for you.
When to apply
Claim within 2 months of the accident; the Director of Agriculture can condone a delay up to 6 months, the Secretary (Agriculture) up to 1 year (cl. 7(a)(v), amended 18.08.2017).
