- Photograph
- Ration card
- Bank passbook (front page) in your name, with IFSC
- Your signature on the receipt (APR), countersigned by the panchayat
- Beneficiary list signed by the panchayat
How to apply
- Your panchayat (PRI body) selects one person per family and signs the beneficiary list (eligibility table, item 1(a)).
- Register on the BMS citizen portal, open “Beneficiary Schemes” and enrol in scheme code CMYJ3 with your 12-digit ration card number (SOP, sections 1–3).
- Upload the documents, choose which animals you have under “Farmer Have at least”, and make the final submission; keep the acknowledgement slip (SOP, sections 3–4).
- The honorarium is paid by DBT to your bank account (eligibility table, item 2).
